Physicals
2024-25 Physical Form - Please print and take to Physician for clearance. Turn into your Coach or Athletic Trainer
After you have completed the Physical Form, please complete the RankOne Online Forms. These are mandatory for athlete eligibility.
Physicals must be completed AFTER April 1st, 2024 to be valid for the 2024-2025 school year.
Concussions
All athletes that are diagnosed with a concussion must follow the Prosper ISD Concussion Policy Guidelines attached below.
